At Horizon Family Dentistry, we aim to make your dental visits smooth and worry-free. Please review our office policies below.

Appointment Scheduling

Horizon Family Dentistry will send you a reminder two days before your scheduled appointment.

Changes and Cancellations

Life can be unpredictable, and we understand that. However, we ask for 2 business days’ notice for cancellations to avoid a fee and to allow us to offer that slot to another patient. If you’re running late, we might be unable to start or finish your treatment in the remaining time. To ensure proper care, we may need to reschedule your appointment.

Dental Emergencies

At Horizon Family Dentistry, we know dental emergencies can happen unexpectedly. Our team will do our best to see you as soon as possible. If you have a dental emergency, please call our office right away.

Insurance Information

If you have dental insurance, please bring the details of your plan for your first visit. We need current insurance information to file your claim. Let us know promptly of any changes. Remember, you’re responsible for any differences in your account. Most insurers allow electronic claims, while others need mailed forms. We’re happy to help with either. We’ll work with you to maximize your benefits. Our fees align with the current Ontario Dental Association guide.

Insured patients, please provide:

Insurance company name

Policy (Group) number, certificate (ID) number, and division number (if any)

Anniversary date, Calendar year, or policy year

Annual maximum benefit per patient per year

Annual fee guide

Coverage percentage for diagnostic, preventative, restorative, endodontic, periodontal services, and significant treatments like crowns, bridges, or dentures.

Payment Policies

Our policy is payment upon service. If that’s not possible, please arrange payment with our office manager before your appointment. We accept Visa, MasterCard, Interac Direct Payment, and Cash.

Full payment is due at your appointment if you don’t have insurance. We provide patients with their account statements and encourage everyone to review and understand their treatment.

 

Privacy Policies

Horizon Family Dentistry takes your privacy very seriously. We adhere to all applicable laws and regulations to protect personal and medical information that we collect, use, or disclose for dental care purposes. Our staff has been trained in our privacy practices, and we store data using secure systems. You are welcome to request to see your information or ask any questions at any time.

We only use your personal information for dental diagnoses, treatment, billing, and insurance. We won’t share your data without your approval. We fully comply with PIPEDA, the federal privacy law. Feel free to ask for our current privacy policy.
